Cancellation Fee Policy:
If it is necessary for you to cancel, please give us 24 hours notice, as it
is difficult to fill appointment slots on short notice. If you cancel with
less notice, you will be charged a failed appointment fee. If you fail to
show up for your first scheduled appointment without notifying us, you will
be charged the full fee for the visit. Cancellation fees are not covered by
insurance. Your full cooperation is appreciated.
Allow 1 hour for your first visit. Please bring the completed new patient
information form with you.
